Having a baby is one of life's most rewarding experiences. Starting or expanding a family is filled with excitement, joy, and plenty of questions. It’s a journey filled with milestones, challenges, and unforgettable moments, and our staff are here to support you every step of the way. 

Thank you for considering us for your baby’s medical home. We  greatly appreciate the trust families place in us. Watching children grow, thrive, and achieve their milestones is why we work in pediatrics, and it’s a privilege we take to heart every day.

What should I bring to my first visit?
  • Bring your driver's license, insurance card and all newborn paperwork filled out.
  • Please bring all hospital records that you were given, including any immunization records and also results from any testing, including hearing screens.
  • You may want to bring an extra blanket, diapers and extra baby clothes in case of any spills!

Before Your First Newborn Appointment

You are welcome to stop by our office and obtain our newborn patient paperwork before you have your baby.

Please bring these completed forms with you to your first appointment.


After your delivery please share our practice name with the hospital so that they can forward your babies records to us.  Once you have delivered your baby you may call our office to schedule their first appointment.
